I/We, hereby warrant that my/our teen is physically and psychologically fit to participate in cooking and meal preparation activities and understand that the choice to participate brings with it the assumption of those risks and results which are part of these activities.
    • I/We understand that photographs and/or videos may be taken of my/our teen of the online setting of this program, and may be used for display, publications and promotion for Andrea Falcone, Registered Dietitian and Certified Fitness Professional.
    • I/We permit Andrea Falcone to collect and use information provided and communicate as needed.
